Please please Please Help with resume?

I have an interview coming up at a Children's Group Home. I need help with formatting and wording my essay. I know it should be simple but I have been self-employed all my life, since 16 now 35. I didn't know I would need a resume it is for a children's Respite Program. I will be a mentoring children as a part of the program. Any helpful tip will be appreciated!
